Income Certificate

Application form for Income Certificate is available at SDO(civil)’s Office. Form can also be downloaded here Income Certificate Application Form

Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  • Pension Payment Order/Certificate from Treasury Officer(for retired government servants
  • Proof of Salary in form of Affidavit (for non-Government applicants)
  • Any one of Residential Proof (EPIC/Ration Card / patta / Certificate from Area Development Council / Electricity Bill/ Telephone Bills)
  • Salary Certificate from Employer in case parent(s)/Applicant are employed
  • Proof of requirement for the Certificate, if applicable
  • Declaration if either of them is not employed

Scheduled Tribe Certificate

Application form for Scheduled Tribe Certificate is available at SDO(civil)'s Office.

Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  • Birth Proof :- SSLC Admit Card, Other Birth Proof (These docs may be accepted if born before 1989)
  • Residential Proof:-EPIC/Voter's Card Electricity Bill Patta Ration Card. Any other Residential Proof
  • Supporting Document :- Self/Parent/Guardian EPIC Card
  • Parent ST Certificate :-ST Certificate of Parent/Siblings(Mandatory for children below 18 years)
  • NOC from clan:-NOC from clan incase of mixed marriage
  • Adoption Proof:-Court Order/Registered Adoption Deed(Mandatory only in case of adopted child)
  • ST Proof-Other State:- ST proof of either parent(Mandatory in case the parents have obtained ST proof from other State
